Change Measurement Plan

The Change Measurement Plan template defines the overall approach for measuring change progress during for the period of the change program.

Typically this includes a statement of objectives, a description of the measurement method (e.g. survey, interviews, focus groups, and third party tools), the target population which will be measured, roles and responsibilities for administering the task and a time-line of activity
